The following form can be used to register a hardcopy or electronic document. An electronic document can be uploaded to the AADC and optionally linked from one of our online databases or metadata records. You can create a cover page from this form that will include all the details below.
- If your document is hardcopy, please attach a printout of the PDF created when you save this form, to the inside cover of the document.
- If your document is electronic, please tick the box marked 'create a cover sheet and attach to the uploaded file' on this form. This means if the document is circulated, the cover sheet is included.
